Bariatric Surgery
Surgical procedures (sleeve gastrectomy, Roux-en-Y gastric bypass, biliopancreatic diversion, adjustable gastric band) that produce substantial sustained weight loss and remission of type 2 diabetes and other obesity-related conditions.

Mechanisms extend beyond restriction/malabsorption to neurohormonal changes (GLP-1, PYY, ghrelin), bile acid signaling, and microbiome shifts. Long-term micronutrient monitoring is essential: B12, iron, folate, vitamin D, calcium, copper, zinc, thiamine, vitamin A, K, and B6. Common complications include dumping syndrome, hypoglycemia, kidney stones (RYGB), and bone loss. The 2022 ASMBS/IFSO guidelines lowered BMI thresholds, recognizing surgery as primary therapy for severe obesity and metabolic disease.
