Calcium-Sensing Receptor (CaSR)
G protein-coupled receptor on parathyroid chief cells and renal tubule cells that detects extracellular ionized calcium and adjusts PTH secretion and renal calcium handling to maintain plasma calcium.

Activating mutations of CaSR cause autosomal dominant hypocalcemia (with inappropriately low PTH); inactivating mutations cause familial hypocalciuric hypercalcemia. Cinacalcet is a calcimimetic that allosterically activates CaSR and is used to lower PTH in CKD-related secondary hyperparathyroidism.
