Step 4 of the NCP: tracking client progress on indicators tied to the assessment and evaluating intervention effectiveness.

Monitoring and Evaluation closes the NCP loop. The RDN selects indicators (typically matching the signs and symptoms documented in the PES statement), compares them against goals or comparative standards, and decides whether to continue, modify, or terminate the intervention. Findings feed back into reassessment and a new NCP cycle.
